Soap Notes

Every note carries the full SOAP structure:

  • Subjective: what the client tells you
  • Objective: what you observe and measure
  • Assessment: your professional evaluation
  • Plan: what you did and what comes next

Plus the date, the services performed, and optional ICD-10 and CPT fields.

Before and after photos

Two separate columns on every note. Take them with the camera, choose from your photo library, import a file, or, on Mac, drag them in. They stay in the order you added them.

Photos are stored in your own iCloud, encrypted, and are never uploaded anywhere else. Two devices can add or delete different photos at the same time without one overwriting the other.

Smart Summary

A button that summarizes an assessment from what you have written in subjective and objective. It runs on Apple's on-device model. The text never leaves your phone and is never sent to any service, ours or anyone else's.

Requires a device capable of Apple Intelligence. See this Apple webpage for compatible models. The button is simply absent on non-compatible models.

PDFs

Export a single note, or a client's entire treatment history, as a properly laid out PDF with the photographs included, for a referral, a records request, or your own file.

Where they live

Notes and photos sync between your iPhone, iPad and Mac through your own iCloud account. We operate no servers and hold no copy. We could not read your notes if we wanted to.

What it does not do

Invalid ICD-10 or CPT codes are not validated. They are currently empty fields for you to use, if needed. Notes attach to a client rather than to a specific appointment.
